Job description

Position Summary:

The Regional Nursing Home Community and Hospital Liason is responsible for developing and implementing strategies to drive census growth and enhance the facility’s presence within hospital systems, physician networks, and the broader community. This role requires strong relationship-building skills with healthcare professionals, case managers, discharge planners, and community partners to position the facility as the provider of choice for post-acute and long-term care.

Key Responsibilities:

Hospital Development

  • Build and maintain strong relationships with hospital case managers, discharge planners, physicians, and social workers.

  • Actively monitor and respond to referral trends within key hospital systems.

  • Conduct regular hospital rounds and identify opportunities for partnership and growth.

Community Development

  • Establish and nurture relationships with community organizations, senior centers, home health agencies, and other referral sources.

  • Plan, coordinate, and participate in community outreach events, health fairs, and educational programs to promote the facility’s services.

  • Represent the facility as a trusted healthcare partner within the local community.

Strategic Growth & Reporting

  • Analyze referral data and market trends to identify opportunities for census development.

  • Collaborate with leadership to create and execute targeted marketing strategies for underutilized programs or specialty services.

  • Provide regular reports on referral patterns, conversion rates, and competitive market activity.

  • Ensure timely follow-up on all leads, inquiries, and referrals.

Qualifications:

  • Minimum of 3–5 years of healthcare marketing, business development, or liaison experience (SNF experience strongly preferred).

  • Established network within local hospitals and healthcare communities is highly desirable.

  • Strong communication, interpersonal, and presentation skills.

  • Ability to work independently while collaborating with an interdisciplinary team.

  • Proficiency in referral tracking systems and Microsoft Office Suite.
