About the Role

Clarvida is seeking a Care Coordinator to provide Targeted Case Management (TCM) and Behavioral Health Home (BHH) services to children and adults with behavioral or mental health diagnoses. In this role, you will meet with clients and families in their homes to assess strengths and emotional, behavioral, and physical needs, develop individualized treatment plans, and connect clients with appropriate community-based resources.

You will engage clients and families regularly to support progress toward goals and collaborate with educational, vocational, medical, and behavioral health providers to deliver coordinated, wraparound services.

Responsibilities

Provide Targeted Case Management (TCM) and Behavioral Health Home (BHH) services
Meet with clients and families in home and community settings
Assess client strengths, needs, and barriers to care
Develop and maintain individualized treatment and service plans
Link clients to community-based resources and supports
Coordinate care with medical, behavioral health, educational, and vocational providers
Engage clients and families regularly to support progress and outcomes
Maintain accurate, timely, and compliant documentation

Requirements

Bachelor's Degree in any field

MHRT-c Required

Valid Driver's License and active automobile insurance
Ability to work independently in the community while collaborating with a multidisciplinary team
