Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

Responsibilities
  • Deliver on-site rounding, primary care, and advanced wound care services.
  • Manage pressure injuries, diabetic ulcers, surgical wounds, skin integrity concerns, and medically complex cases.
  • Lead chronic disease management and preventative care initiatives.
  • Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
  • Help reduce avoidable hospitalizations through proactive clinical intervention and follow-up care.
  • Communicate effectively with families, caregivers, and interdisciplinary teams.
  • Complete documentation, medication management, diagnostics, and lab coordination.
Qualifications
  • Active Minnesota Physician Assistant (PA-C) license in good standing.
  • Experience in wound care, geriatrics, primary care, internal medicine, family medicine, or post-acute care preferred.
  • Strong clinical judgment and confidence managing medically complex patient populations.
  • Valid driver's license and reliable transportation.
