Job description

Job Title: Home Health LPN

Location: Johnson County Hospital, Tecumseh, NE

Department: Home Health Administration

Reports To: Home Health Director

Position Summary:

To performs duties under general medical/nursing supervision and direction of patient's provider, Home Health Director and Home Health RN Case Manager. Performs home visits for the purpose of providing Home Health services according to the written plan of care. Partners and participates with the RN Case Manager in the ongoing review and facilitation of the care plan, patient treatments, including request of interventions, status of and goal setting as well as family/caregiver needs for the patients that are assigned. Responsible to maintain and facilitate compliance and regulatory process as identified by CMS and JCHH.

Duties & Responsibilities:

  • Liaison between patient, family/caregiver, RN Case Manager and physician
  • Partners with RN Case Manage to plan and facilitate patient care according to each patients needs.
  • Assists with weekend patient care coverage as designed by the Director.
  • Provides routine care for patients including head to toe assessments, vitals and mental health evaluations.
  • Able to provide observation and reporting of patient's health or changes to RN Case Manager, consulting physicians and providers.
  • Assist Physicians and Registered Nurses with care and treatments ordered.
  • Communicates interventions and goals with patients and their family members or caregivers as prescribed by the Plan of Care.
  • Administers oral and injectable medications, refills, reconciles medications with each and every visit.
  • Preforms standard diagnostic procedures including phlebotomy/venipuncture or other lab requests able to be completed in the home.
  • Provides wound care and skin management.
  • Monitors, changes provide cares for catheters or appliance including PICC dressing changes, LVAD dressing changes, Pluer-evac, Ostomy, ileostomy, Foley, Suprapubic, Flush IV PICC lines as ordered and assist with IV infusions.
  • Reviews medical charts within agency EMR, JCH Cerner to maintain continuity of care and follow up.
  • Communicates with families and caregivers.
  • Participates in Agency Chart auditing as directed.
  • Understands the guidelines which determine the compliance, regulatory requirements of CMS and the payment mode of the patient.
  • Accurate and timely completion of all required documentation of his/her patients.
  • Has knowledge of the state laws governing the scope of nursing practice.
  • Works with area hospitals, nursing homes and physicians regarding discharge planning as needed.
  • Performs any other duties deemed necessary by the Home Health Director.

Qualifications & Skills:

  • Licensure as a Licensed Practical Nurse in the State of Nebraska, completes necessary CEU's, maintains unencumbered license.
  • Employed by the Johnson County Home Health (JCHH) a service provided to the area and surrounding communities by Johnson County Hospital (JCH).
  • BLS Certification.
  • 1-2 years recent nursing experience preferred.
  • Meets all requirements under JCH and JCHH policies for Home Health Nurse - LPN.
  • Professional or personal experience supporting individual with disabilities, mental illness, or challenging behaviors highly preferred.
  • 3 + years customer service experience, preferably in human services and possess/demonstrates a commitment to customer service.
  • Maintains a private vehicle and has insurance coverage for he/her own transportation for making home visits.
