Job description

Position Summary
Responsible for organizing, planning and managing the patient medical records system for Eagleville Hospital inpatient program. This position reports to the Chief Legal and Risk Officer.
Objectives / Responsibilities
  • Maintain patient medical record system for the hospital in accordance with the policies of the hospital as well as The Joint Commission (TJC), Department of Drug and Alcohol Programs (DDAP), Department of Health (DOH), Office of Mental Health and Substance Abuse Services (OMHSAS), State and Federal law and other regulatory agencies. Participate in surveys through pre-survey preparation, printing charts for review, interviews, and follow-up reports as scheduled.
  • Maintain patient confidentiality and patient privacy in accordance with regulatory agencies.
  • Organize and plan the work of the department on a daily basis.
  • Process all patient information telephone calls and messages as received.
  • Process all written requests/releases for patient information as received.
  • Attend and represent the department on committees and at meetings as scheduled.
  • Serve as the Chairperson of the Medical Record Committee. Prepare agendas, schedule meetings, complete minutes on a quarterly basis.
  • Provide consultation to all Eagleville Hospital programs regarding organization, maintenance and content of patient record system as needed.
  • Develop and maintain the policy/procedure manual for the department on an annual basis. Inform the clinicians/supervisors of revisions and provide assistance through the Medical Records Committee.
  • Conduct chart audits to assure that all appropriate documentation is included in the medical record and that the content is complete and accurate, has sufficient detail to reflect the treatment and services provided, the patient's course through treatment, the goals of treatment, and to support billing and coding functions.
  • Provide documentation training as necessary to providers and caregivers who fail to meet required expectations.
  • Serve as custodian of patient records in response to subpoenas and court orders as needed.
  • Supervise staff and complete competency evaluations and training plans on staff during the year.
  • Interface with Coders and assure that all pertinent charts are appropriately coded. Participate in the Revenue Cycle and with Compliance as needed and requested.
  • Participate in the improvements and ongoing configuration of the electronic health record, including being a member of the EHR Steering Committee.
  • Provide departmental training to staff as new duties and responsibilities are assigned.
  • Provide required additional compliance training to staff on an annual basis.
  • Attend and complete all mandated trainings within the required timeframes.

  • Operate the department within budgetary guidelines.
  • Other duties as assigned.

Bachelor's Degree or greater, Registered Health Information Administrator (RHIA) Certification.
Qualifications
