Job description

Active Home Health is seeking a PRN Home Health RN to serve patients throughout Utah County. This role offers exceptional flexibility-perfect for nurses looking to pick up extra hours alongside a full-time position, or for parents who want to work while their kids are in school. We are looking for caring, professional and self-motivated nurses who enjoy meaningful, one-on-one patient care. Job Requirements:

  • Visit and assess patients to complete an initial comprehensive OASIS or assessment and ongoing assessments through end of care.
  • Chart all clinical documentation in our EHR software
  • Create and manage patient plan of care
  • Educate and give instruction to patient, family, and caregivers as appropriate per patient care plan.
  • Case manage the patient episode or benefit period, ensuring that all cares are performed and documented
  • Provides medication education and management per physician order
  • Works hand-in-hand with the IDT to ensure patient goals are met and appropriate outcomes are achieved
  • Shares on-call duties
  • Maintains strong communication with patient, family, caregivers, and the interdisciplinary teams

Job Qualifications:

  • Registered nurse with current license to practice professional nursing in the state of Utah
  • Polite and compassionate
  • Excellent observation, problem solving, verbal and written communications
  • Maintains a current CPR certification
  • 1 year Home Health or Hospice experience preferred
  • Must be a licensed driver with an automobile that is insured, registered and in good working order
  • Good communication and smartphone skills
  • Ability to pass a background check
