Pay Range: $22.00-$26.00 per hour

Schedule: Monday-Friday 8:00am-5:00pm


About the Job

Seasons is a comprehensive behavioral health center offering a broad range of psychiatric and behavioral health services to communities in the Northwest Iowa region. We're currently seeking a full-time Care Coordinator. The Care Coordinator will help coordinate service provision for providers, facilitating communication between providers, and ensuring individuals receive the appropriate care for their individual needs.

Requirements

  • A Bachelor's degree in social work, human services, or a related field is required
  • Experience providing care coordination services for individuals with behavioral health concerns is preferred but not required
  • An active Iowa Bachelor's of Social Work (BSW) license is preferred but not required
