MDS Coordinator

The Medical Suites at Oak Creek is seeking a compassionate, professional MDS Coordinator to join our team. As our MDS Coordinator, you will play a key role in assessing resident health, collecting and analyzing data, and developing individualized care plans that align with each resident's needs and expectations. This position is ideal for an experienced nurse who enjoys a combination of clinical and administrative responsibilities, with a focus on quality care and compliance.

This is a Part-Time position

Job Requirements:

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing
  • Previous MDS experience in long-term care is preferred
  • Strong communication, organization, and analytical skills

Job Responsibilities:

  • Conduct comprehensive assessments of residents
  • Develop and update resident care plans
