Job Title: Certified Nursing Assistant (CNA) or Home Health Aide (HHA)

Three Full Time Positions 7:30am - 4:30pm or 8am to 5pm Monday - Friday + Every Other Weekend

40 hours weekly - *Must complete job requirements as outlined in On-boarding & Orientation

Job Summary:
We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our healthcare team. As a CNA, you will provide direct patient care and support under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN). Your primary responsibilities will include assisting patients with daily activities, monitoring vital signs, maintaining patient records, and collaborating with the healthcare team to ensure the provision of high-quality patient care.

Responsibilities:
- Provide direct patient care visits in accordance with the individualized care plan, including assisting with bathing, dressing, grooming, and toileting.
- Assist patients with mobility by helping them with walking, transferring, and repositioning.

- Flexible to travel to 3-5 patients home per day; drive time included in work hours

- Measure and record vital signs including blood pressure, pulse rate, temperature, and respiratory rate.

- Monitor patients’ conditions and report any changes to the nursing staff.
- Observe patients for physical or emotional changes and promptly report any concerns to the nursing staff.
- Ensure accurate documentation of patient care activities, including maintaining and updating patient records as per facility protocols.
- Collaborate with the healthcare team, including nurses, and other support staff, to ensure the provision of comprehensive patient care.
- Assist with the collection of laboratory samples and perform basic diagnostic tests as required.
- Maintain a clean and organized patient environment, including regularly changing bed linens, emptying trash, and ensuring cleanliness and infection control protocols are followed.
- Assist in the implementation of recreational activities and therapy programs to promote the physical and emotional well-being of patients.
- Adhere to all established policies and procedures, nursing standards, and government regulations.
- Participate in continuing education programs to enhance knowledge and skills.

Requirements:
- Certified Nursing Assistant (CNA) certification or HHA Test/Assessment.
- High school diploma or equivalent.
- Previous experience as a CNA or in a similar healthcare role is preferred, must GPS clock in/out, client signed paper timesheet

- Have sense of urgency when caring for patients and traveling in-between patient homes

- Excellent communication and interpersonal skills.
- Compassionate and patient-centered approach to care.
- Ability to prioritize tasks and manage time effectively.
- Strong attention to detail and accuracy in documentation.
- Knowledge of infection control protocols and safety measures.
- Physical stamina to lift, push, and pull heavy equipment and assist patients with mobility.
