Licensed Practical Nurse (LPN)

Department: Nursing

Reports To: Director of Nursing

FLSA Status: Non-Exempt

Location: Lincoln Village Healthcare Center, Lincoln, IL


PAY: $35.00 per hour ($10,000 Sign-On Bonus and Mileage Reimbursement available for Full-Time)

Hours: 12- Hour Shifts (6-6) All shifts available.

What We Are Looking For:

The Licensed Practical Nurse (LPN) provides nursing care to residents under the supervision of a Registered Nurse (RN) and in accordance with federal, state, and facility standards. The LPN is responsible for administering medications, performing treatments, monitoring residents’ health status, and supporting the overall care plan to promote optimal health outcomes.

What You Will Do:

Resident Care

  • Administer medications and treatments as prescribed, in accordance with facility policies and state regulations.

  • Monitor residents’ health status, observe for changes in condition, and promptly report findings to the RN or physician.

  • Assist residents with activities of daily living (ADLs) as needed.

  • Ensure residents receive appropriate nutritional support and hydration.

Documentation & Communication

  • Accurately document all care provided, medications administered, and resident observations in the medical record.

  • Participate in care plan development and interdisciplinary team meetings.

  • Communicate effectively with residents, families, and healthcare team members.

Safety & Compliance

  • Follow infection prevention and control procedures, including proper use of personal protective equipment (PPE).

  • Maintain a safe environment for residents, visitors, and staff.

  • Comply with all state, federal, and facility regulations, including HIPAA.

Leadership & Supervision

  • Direct and monitor the work of Certified Nursing Assistants (CNAs) to ensure quality care.

  • Provide training, guidance, and feedback to CNAs as needed.

  • Assist in onboarding and orientation of new nursing staff.

What You Will Bring:
  • Current, valid Illinois LPN license in good standing.

  • Graduation from an accredited practical nursing program.

  • Prior experience in long-term care or a similar healthcare setting preferred.

  • Strong clinical skills and ability to respond quickly in emergency situations.

  • Excellent communication, leadership, and organizational skills.

Physical Requirements:
  • Ability to lift/push/pull up to 50 lbs and assist with resident transfers.

  • Frequent standing, walking, bending, and reaching.

  • Ability to work in a fast-paced environment with potential exposure to infectious diseases.

Work Environment:

LPNs work in a healthcare setting with frequent resident contact, exposure to body fluids, and potential exposure to infectious diseases. This position requires adherence to safety and infection control protocols and the ability to work collaboratively within a healthcare team.
