Job description

INTRODUCTION:

Central Neighborhood Health Foundation is a Federally Qualified Healthcare Center (FQHC) committed to the Triple Aim as described by the Institute for Healthcare Improvement. Improving the US health care system requires simultaneous pursuit of three aims: 1) improving the experience of care, 2) improving the health of populations, and 3) reducing per capita costs of health care. Preconditions for this include the enrollment of an identified population, a commitment to universality for its members, and the existence of an organization (an "integrator") that accepts responsibility for all three aims for that population.


SUMMARY:

The Associate Behavioral Health Therapist will provide outpatient mental health services, including psychotherapy using evidence-based practices as well as case management to eliminate mental health barriers to employment and enhance emotional well-being in order to achieve financial security, stability, and self-sufficiency.


ESSENTIAL DUTIES AND RESPONSIBILITIES include but are not limited to the following functions:

  • Provides psycho-social assessments of patients to identify emotional, social, and environmental strengths and/or problems related to their diagnosis, illness, treatment, and/or life situation
  • Provides patients with knowledge regarding the patient's diagnosis, illness, and/or treatment
  • Familiarity with hospital, medical center, and/or health system resources to access and provide for patient care needs
  • Participate in case review meetings
  • Documents assessments, plans, interactions, and interventions according to CNHF system guidelines and standards.
  • Plan, develop, and facilitate educational activities for patients, including but not limited to presentations, workshops, goal-setting and management, addressing techniques for health improvement, trouble areas, etc.
  • Remain current with social work and health care developments
  • Stays in compliance with all HIPAA regulations
  • Performs other duties as assigned

EDUCATION AND/OR EXPERIENCE:

  • Computer literacy required (Microsoft Office Suite)

  • Knowledge of laws and ethical standards of clinical care
  • Master’s Degree in Social Work
  • Current CA clinical license (ACSW)
  • Minimum of two years in clinical experience post master’s degree, preferably working in a community clinic setting or a mental health agency that serves a wide variety of clients
  • Exceptional interpersonal, written, and oral communication skills
  • Able to work independently and in a team; self-directed
  • Bilingual (English/Spanish) preferred, not required
