Join a growing clinical team dedicated to providing high-quality, patient-centered care within post-acute and long-term care settings. We are seeking a motivated Nurse Practitioner or Physician Assistant to manage the ongoing medical needs of residents in the Bradenton area.

This role offers a collaborative environment and a strong learning curve. Whether you are an experienced provider or a motivated new graduate, we provide the supportive training necessary to excel in clinical decision-making and interdisciplinary care.

Key Responsibilities

  • Patient Management: Conduct comprehensive evaluations and follow-up visits for residents in skilled nursing facilities (SNFs).
  • Clinical Strategy: Develop, implement, and adjust individualized treatment plans based on patient needs.
  • Collaboration: Work closely with physicians, nursing staff, therapists, and facility leadership to ensure cohesive care.
  • Documentation: Maintain accurate and timely patient records using Electronic Health Record (EHR) systems.
  • Care Coordination: Participate in interdisciplinary care planning meetings to optimize patient outcomes.

Qualifications & Requirements

  • Licensure: Valid license as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Florida.
