About Us

Baldwin Family Health Care (BFHC) is a Federally Qualified Health Center (FQHC) committed to delivering accessible, high-quality care to underserved communities across Michigan. Our pharmacy team plays a vital role in improving health through clinical expertise, education, and affordable medication access for eligible patients.


Position Summary

Baldwin Family Health Care is expanding clinical pharmacy services! This is a unique opportunity for a pharmacist passionate about working directly with patients in both community and ambulatory care settings. You’ll collaborate with an interdisciplinary healthcare team optimize therapies, support chronic disease management, and improve outcomes. This position is evenly split between our family medicine clinic and outpatient pharmacy. We’re seeking a candidate excited to contribute to the continued growth of our ambulatory care services.

Key Responsibilities – Staff Pharmacist

  • Deliver dispensing and clinical services, including immunizations, MTM, and patient counseling
  • Promote safe, effective medication use and adherence
  • Partner with providers to address therapy concerns and optimize regimens
  • Document encounters in pharmacy and clinical systems
  • Mentor and support pharmacy technicians and student pharmacists


Key Responsibilities – Clinical Pharmacist

  • Partner with BFHC providers and staff to deliver evidence-based, cost-effective care
  • Conduct patient visits to educate on medications and disease management
  • Manage chronic conditions under collaborative practice agreements (e.g., diabetes, hypertension, hyperlipidemia, anticoagulation, COPD, tobacco cessation)
  • Participate in a multidisciplinary team of physicians, advanced practice providers, nurse care managers, behavioral health specialists, and community health workers
  • Respond to drug information requests, complete comprehensive medication reviews, and resolve therapy-related problems
  • Assist in developing and evaluating clinical pharmacy services


Qualifications

  • Doctor of Pharmacy (PharmD) or equivalent degree
  • Completion of PGY-1 Pharmacy Residency or comparable ambulatory care experience preferred
  • Active pharmacist and controlled substance license in Michigan
  • Strong interpersonal and communication skills
  • Commitment to professionalism and working with diverse populations
  • Ability to thrive in a multidisciplinary team environment
  • Outpatient or retail pharmacy experience required
  • Experience with the 340B Drug Pricing Program preferred
  • Familiarity with Pioneer Rx and NextGen electronic health record preferred
