HOME HEALTH CLINICAL MANAGER (RN) - OFFICE & FIELD

FULL-TIME / SALARY POSITION

Elite Care Management is seeking an experienced Home Health Clinical Manager (RN) to support our Indiana office. This role includes both office-based clinical leadership and field responsibilities to ensure high-quality, compliant patient care.

Elite Care Management is a privately owned home health care company with over 26 years of experience in providing compassionate and personalized care to patients in their homes.

Responsibilities:

  • Provide clinical oversight and case management for home health patients
  • Review OASIS, plans of care, and clinical documentation
  • Supervise and support field clinicians (RNs, LPNs, therapists, CNAs/HHAs)
  • Perform supervisory and patient field visits as needed with rotating on-call
  • Ensure compliance with Medicare, Indiana State Regulations, and accreditation standards
  • Participate in QAPI, infection control, and quality improvement activities

Requirements

  • Active Indiana RN license
  • 2+ years of home health experience preferred
  • Clinical leadership or supervisory experience preferred
  • Knowledge of OASIS and Medicare guidelines is a plus
