This position offers a remote work setting, enabling the LCSW to maintain a flexible and adaptive schedule. While primarily home-based, work will involve regular collaboration with a multidisciplinary team consisting of psychiatrists, psychologists, nurses, and case managers through virtual platforms. • Conduct thorough mental health assessments, including detailed psychosocial evaluations, diagnostic interviews, and risk assessments to identify patients' needs effectively.
• Develop tailored treatment plans in collaboration with patients, integrating their objectives and cultural considerations to enrich therapeutic engagement.
• Employ evidence-based therapeutic modalities, including cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), in diverse settings such as individual, couple, family, and group therapy sessions.
• Provide timely crisis intervention services, ensuring immediate support for patients experiencing acute distress while coordinating with healthcare colleagues for seamless care continuity.
• Maintain comprehensive and accurate clinical documentation, including treatment plans, progress notes, and discharge summaries, compliant with clinic policies and industry regulations.
• Offer psychoeducation to patients and their families, empowering them with the necessary knowledge and skills to actively manage their mental health conditions and contributing to an informed support network.
• Regularly assess treatment effectiveness by conducting outcome evaluations and adjusting treatment plans based on therapeutic progress and patient feedback.
• Stay abreast of the latest research, best practices, and emerging trends in mental health, and actively incorporate this knowledge into clinical practice to enhance service delivery.
• Uphold the highest standards of ethical practice by maintaining patient confidentiality and adhering strictly to established ethical guidelines.

The LCSW will primarily serve a diverse range of patients, including individuals from varied cultural backgrounds and age groups who are experiencing mental health challenges. This role requires sensitivity and adaptability to build rapport and trust with clients undergoing complex life situations or crises.

• A Master’s degree in Social Work (MSW) is required for this position.
• An active state license as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Strong ability to function both independently and collaboratively within a multidisciplinary team framework.
• Exceptional interpersonal and communication skills, with a proven track record of establishing fruitful relationships with a diverse clientele.

While a master's degree and active LCSW license are mandatory, candidates with prior experience in community health settings, crisis intervention, and evidence-based therapeutic modalities are preferred. Familiarity with electronic health record (EHR) systems is also advantageous.
