We believe managing clinical risk is more than identifying problemsโ€”it is about preventing harm, strengthening clinical practices, supporting our teams, and ultimately improving outcomes for those entrusted to our care.

The Clinical Risk Manager serves as a trusted clinical consultant and strategic resource to our skilled nursing communities. This position partners with facility and regional leadership to identify and mitigate clinical risk, recognize trends, strengthen quality improvement efforts, and promote a culture of safety, accountability, and continuous improvement.

This is a highly visible corporate role that works across multiple communities and collaborates with Administrators, Directors of Nursing, Medical Directors, Regional Clinical Specialists, Regional Vice Presidents, Operations, Quality, and Legal. The Clinical Risk Manager brings clinical expertise and a proactive approach to help communities understand risk, address root causes, and implement sustainable solutions.

Clinical Risk Assessment & Identification
  • Conduct comprehensive clinical risk assessments through baseline data collection, trend analysis, and review of clinical and quality indicators.
  • Evaluate areas of potential risk, including high-priority centers, survey activity, quality measures, adverse events, quality improvement initiatives, and applicable monitoring tools.
  • Identify emerging and existing clinical risks and proactively communicate findings and recommendations to facility and regional leadership.
  • Serve as a clinical advisor and resource to Center Quality Assurance and Performance Improvement (QAPI) committees.
  • Review clinical trends and data to identify opportunities to improve resident outcomes and reduce preventable risk.
  • Partner with facility leadership to develop and implement appropriate interventions based on identified risks.
Loss Control & Root Cause Analysis
  • Assist with the investigation of unusual occurrences and other priority clinical events.
  • Support root cause analysis and development of corrective action plans.
Risk Reduction & Education
  • Serve as a consultant and educator to facility teams regarding clinical risk prevention, regulatory compliance, quality improvement, and resident safety.
  • Provide education designed to strengthen clinical practices and promote consistent adherence to corporate programs, policies, and processes.
  • Help facility leaders understand the relationship between quality outcomes, regulatory compliance, clinical practices, and risk management.
  • Develop or facilitate targeted education based on identified trends, recurring risks, and individual community needs.
  • Promote a proactive culture in which teams identify and address potential risks before they result in adverse outcomes.
Qualifications
  • Current Registered Nurse (RN) license in the State of Texas in good standing
  • Strong knowledge of federal and Texas long-term care regulations and skilled nursing clinical practices.
  • Must reside within Regency Integrated Health Services' service area.
  • Willingness and ability to travel extensively throughout assigned areas of Texas.
Preferred Experience & Certifications
  • Five or more years of long-term care experience, with preference given to candidates with prior Director of Nursing or regional clinical leadership experience.
  • Experience with skilled nursing regulatory compliance, survey processes, QAPI, clinical investigations, and risk management.
  • Licensed Risk Manager (LRM) certification or eligibility to obtain certification preferred.
  • Certified Legal Nurse Consultant (CLNC) certification or eligibility to obtain certification preferred.
Travel & Schedule

This position requires regular travel throughout assigned communities and other areas of Texas as needed. The Clinical Risk Manager must maintain flexibility to accommodate changing priorities, community needs, investigations, meetings, and organizational initiatives.
