This role is primarily remote, providing the opportunity for flexibility while still promoting a supportive team atmosphere through regular virtual meetings and case conferences. • Conduct thorough intake and psychosocial assessments to understand each patient’s individual experiences and needs.
• Collaborate with the Chemical Dependency Research Panel/Social Treatment team to develop, present, and fine-tune treatment plans, ensuring that strategies are evidence-based and tailored for each patient.
• Participate actively in clinical case conferences to discuss patient progress, share insights, and receive feedback to inform interventions.
• Facilitate educational lectures focusing on chemical dependency, tailored to families and patients, providing vital information and support.
• Provide comprehensive counseling services, including individual, group, and family therapy, to address issues concerning chemical dependency and related concerns.
• Manage case management services that involve resource coordination and referrals to community support systems, ensuring patients and families know their options.
• Offer specialized therapy sessions for adolescents, significant others, families, and multi-family units, addressing the complex dynamics of addiction and its effects on relationships.

This role primarily serves patients within the hospice care spectrum, including individuals facing terminal illnesses and their families. You will work closely with a diverse clientele, requiring a robust ability to provide empathetic support and skilled interventions that address not only the psychological aspects of end-of-life care but also the social implications for their loved ones.

• A master’s degree in social work (MSW) or a related field is required for this position.
• The candidate must hold a valid LCSW license in the state of Oregon.
• A minimum of two years of post-graduate, supervised experience in addiction medicine therapy within the last five years is essential.

• Experience working in hospice or palliative care settings, particularly with patients undergoing various stages of illness.
• Familiarity with evidence-based therapeutic practices for addiction and mental health disorders.
• Ability to engage in family therapy with a focus on resolving conflicts and improving communication within family systems affected by addiction.

The salary for this role ranges from USD 115,000 to 130,000 annually, commensurate with experience and qualifications.
