Reports To: Director of Hospice Operations

At Vitality Hospice, we are dedicated to providing compassionate, patient-centered hospice care that supports patients and families with dignity, respect, and comfort. We are seeking a compassionate, organized, and detail-oriented Intake Coordinator to join our growing team.

As the Intake Coordinator, you will play a vital role in ensuring a smooth and timely admission process by coordinating referrals, communicating with healthcare providers, patients, and families, and supporting our clinical team. Position Summary

The Intake Coordinator serves as the primary point of contact for incoming hospice referrals and is responsible for coordinating the intake process from referral through admission. This role requires excellent communication, organization, attention to detail, and the ability to provide exceptional customer service while maintaining compliance with hospice regulations and HIPAA standards.

Key Responsibilities
  • Receive and process incoming referrals via phone, fax, email, and electronic referral platforms.
  • Communicate with hospitals, physician offices, skilled nursing facilities, case managers, and other referral sources to obtain required documentation.
  • Collect, verify, and accurately enter patient demographics, insurance information, physician orders, and clinical documentation into the EMR system.
  • Coordinate with the clinical and admissions team to ensure timely patient admissions and scheduling.
  • Verify insurance eligibility and ensure all required documentation is complete prior to admission.
  • Maintain accurate referral records and monitor the status of pending referrals.
  • Communicate professionally and compassionately with patients, families, referral sources, and internal staff.
  • Maintain confidentiality and comply with HIPAA and all company policies.
  • Assist with administrative duties related to the intake process.
  • Collaborate with clinical leadership to ensure an efficient, patient-focused admissions experience.
Qualifications
  • High school diploma or GED required; Associate's or Bachelor's degree preferred.
  • Previous experience in hospice, home health, healthcare intake, admissions, scheduling, medical office, or customer service preferred.
  • Excellent communication, organizational, and multitasking skills.
  • Strong attention to detail and ability to work in a fast-paced environment.
  • Tech-savvy with the ability to quickly learn new software and healthcare technology platforms. Proficient in Microsoft Office and comfortable navigating electronic medical record (EMR) systems and multiple computer applications simultaneously.
  • Knowledge of hospice or home health regulations is a plus.
