Job description

Mountain Park Health Center is looking for Clinical Pharmacists to join our well-established and accredited group of clinical pharmacists who provide high-quality patient care and services to patients at one of our many outpatient clinics. As a member of the integrated health care team, the Clinical Pharmacist collaborates with the medical provider to provide specialized pharmaceutical services to patients s to improve drug usage and therapeutic outcomes for Mountain Park Health Center. The position facilitates medication management through medication reconciliation, evaluation of adherence, effectiveness, and toxicity, and is a resource to providers and members of the care team on issues concerning drug therapy, and side effects, as well as assisting in appropriate doses.
