How you'll contribute

A Outcomes Managerwho excels in this role:

  • Coordinate PPD data collection with the interdisciplinary team.
  • Ensure supportive documentation for QIs is complete in the medical record.
  • Complete admission and discharge IRF-PAIs within CMS-designated timeframes.
  • Abstract required information from medical records and summarize on the IRF-PAI.
  • Enter data into the CMS-accepted software/program within designated timeframes.
  • Maintain and update the IRF-PAI log; verify accuracy against the patient location roster.

What we're looking for:
  • Clinical degree with current license to practice as an OT, PT, RN, SLP, or MSW
  • QI competency required
  • Working knowledge of state and federal guidelines; familiarity with healthcare workflows and documentation required
  • Basic computer and data entry proficiency required
  • Strong clinical assessment/analysis, effective verbal and written communication, excellent interpersonal skills, and ability to collaborate across disciplines
