Job/Role Description:

Jaan Health is seeking a proactive and mission-driven Clinical Operations Manager to lead and support our team of Clinical Care Coordinators (LPNs and MAs) and Clinical Care RNs delivering high-quality care management services. As a key member of the Clinical Operations team, this role is responsible for overseeing day-to-day clinical operations, supervising the care team's performance, managing patient care workflows, and ensuring alignment with Advanced Primary Care Model (APCM) initiatives.

This role will directly report to the Director of Clinical Operations and will work cross-functionally with Product, Client Success, Service Delivery, and Leadership teams to ensure high performance, quality outcomes, and strong patient engagement.

Key Responsibilities:
  • Oversee hiring, onboarding, training, performance management, as well as directly manage and provide ongoing coaching for a team of RNs, LPNs, CMAs, and Clinical Care Coordinators.
  • Support care team members in managing triage, care coordination, and clinical interventions and manage staff schedules, workload distribution, and clinical escalations.
  • Ensure consistent and high-quality delivery of care management services aligned with APCM protocols, value based care goals, and regulatory requirements.
  • Monitor and report on key team metrics including outreach volume, patient engagement, documentation accuracy, and care plan adherence.
  • Identify and implement process improvements that improve patient experience and clinical outcomes.
  • Partner with internal teams (Product, Client Success, Service Delivery and Engineering) to improve workflows and platform usage.
  • Support client-facing initiatives, including clinical quality improvement and program implementation initiatives, provider coordination, and performance reviews.
Requirements:
  • Bachelor's degree in Nursing with an active, unrestricted RN compact license.
  • 12+ years of clinical experience, including 1โ€“2 years in a leadership or operations management role.
  • Experience managing RNs, LPNs, and CMAs, in ambulatory, chronic care, or value-based care environments, with demonstrated success managing a remote/hybrid team.
  • Strong understanding of Care Management, Care Coordination, CCM/APCM workflows, CMS regulations, and quality programs.
  • Proficiency in EHR systems (Epic, Cerner, eCW or Veradigm preferred).
  • Highly organized with excellent communication, leadership, conflict resolution skills who is tech-savvy and comfortable using digital platforms, analytics dashboards, and communication tools.
  • Access to a secure, private at home work environment with secure home internet and ability to travel 20% for team or client meetings.

Preferred requirements:

  • Masters degree in Nursing, Healthcare Administration, Public Health or Business Administration highly preferred.
  • Previous experience managing APCM, CCM, Population Health and/or Value Based Care programs.
  • Bilingual (Spanish speaking) a plus.
