Job Summary
Supports the Director of Patient Care Services and clinical team to ensure high-quality patient care, efficient staff coordination, and compliance with all regulatory requirements in a home health setting.
Key Responsibilities
  • Coordinate clinician schedules and patient assignments to maximize efficiency and coverage
  • Monitor productivity and ensure clinicians meet visit expectations
  • Support onboarding, training, and ongoing staff development
  • Promote strong interdisciplinary communication and teamwork
  • Ensure compliance with state, federal, and Medicare regulations
  • Conduct chart audits and support quality assurance initiatives
  • Communicate with physicians, patients, families, and pharmacies
  • Assist field staff with orders, lab follow-ups, and care coordination

Qualifications
Required:
  • Active clinical license
