• Conduct thorough clinical assessments to evaluate patients' psychosocial needs and formulate effective treatment plans tailored to individual circumstances.
• Administer evidence-based therapeutic interventions, including individual and family counseling, to assist clients in coping with grief, loss, and the emotional turmoil associated with terminal illness.
• Maintain accurate and timely clinical documentation that complies with regulatory standards, facilitating continuity of care and supporting therapeutic outcomes.
• Collaborate closely with a multidisciplinary treatment team, including physicians, nurses, and allied health professionals, to ensure comprehensive care for patients and their families.
• Provide psychoeducation, emotional support, and resources to patients and their families, empowering them to navigate the complexities of end-of-life issues.
• Stay abreast of current research and advancements in social work and mental health treatment methodologies to enhance clinical skills and improve patient care.

In this role, you will primarily engage with patients in hospice care, addressing their unique mental health needs as they face end-of-life challenges. You will also support families, helping them process grief and offering strategies to cope with their loved one’s circumstances. Your expertise will directly impact their quality of life, facilitating a peaceful and dignified transition.

• A Master’s degree in Social Work from an accredited institution is mandatory.
• A valid LCSW license issued by the state of California is required to practice in this role.
• In-depth knowledge of solution-focused psychotherapy techniques and an ability to apply them effectively in clinical practice.
• Strong communication and interpersonal skills, enabling you to build rapport with patients, families, and colleagues.

• Experience in hospice, palliative care, or geriatric social work is highly desirable.
• A background in crisis intervention and comfort with discussing end-of-life topics will enhance your effectiveness in this role.
• Previous experience working in a multidisciplinary team-focused environment can provide an advantage.
