Job Title: Outpatient Nurse (RN or LPN)

Job Code: HCT031/Licensed Practical Nurse 2 - T7002/Health Care Assistant

Shift: Full-Time (32 hours per week on average)

Job Hours: 8:30am - 5:00pm, with an option for flexible, extended office hours and/or weekend coverage if needed.

Base Location: Onsite - Conyers, GA

Division/Department/Program: Rockdale Outpatient Clinic

View Point Health proudly provides outpatient treatment to children, adolescents and adults. At the outpatient clinics, we promote overall health and quality of life for individuals, youth and families through evidenced-based therapies and medication management to reduce the impact of mental illness, behavior challenges, and substance use.

The Outpatient Nurse provides professional nursing care in an outpatient clinic for clients being treated for Mental Health &/or Substance Use Disorders.

  • Performs initial assessments of adult and/or child/adolescent clients presenting with mental health or alcohol/drug problems.
  • Interviews client/family, takes vital signs, performs necessary routine tests & phlebotomy services, as needed.
  • Documents observations, medical history, physical and psychosocial examination results, and initial analysis of diagnostic data.
  • Formulates individualized nursing treatment plan based on assessments, treatment plan, and the client's level of need and functional ability.
  • Performs direct client care including medication management, administration of injections, and instruction to the client on self-care. Observes and records in the client's progress notes responses to treatment and any new symptoms or other changes in the client's condition.
  • Notifies the medical provider or clinician of significant side effects or other adverse reactions observed or reported. Makes suggestions to the treatment team regarding any needed changes to the client's treatment plan, adjusts nursing care plan accordingly, and secures necessary authorizations and re-authorizations.
  • Maintains medical records that meet all applicable standards. Writes clear, concise progress notes that relate directly to the service plan and properly document treatment outcomes.
  • May provide group, or individual training and supportive counseling to mental health clients and their families as needed. Provides medical and symptom psychoeducation. Linkage and care coordination with other medical providers/PCPs.

Minimum Qualifications:

  • Georgia license as RN or LPN.
  • Previous experiences in psych nursing, or working in Community Mental Health (CSB)
  • Must pass criminal background check. Pre-employment drug screen required for positions in alcohol/drug programs

Preferred Qualifications:

  • Experience with Carelogic or other ECR/EMR
  • Bilingual a plus - with additional supplement

Requirements/Competencies:

  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
  • Candidates for selection must pass a criminal background check (including fingerprinting).
  • Must pass a pre-employment drug screen and subject to random drug screens.

Note: Some positions may require a valid driver's license.
