Care Coordinator

Synergy HomeCare of Northeast Chicago is seeking an experienced Care Coordinator / Case Manager with IDOA, CCP, CCU, or Managed Care Organization (MCO) experience to join our growing team.

This role is ideal for professionals currently working in Care Coordination Units (CCUs), Area Agencies on Aging (AAA), Medicaid MCOs, or senior services organizations who are looking to transition into a provider-side leadership role.

You will play a key role in managing client intake, coordinating care services, and building strong relationships with referral partners across Cook County.

Key responsibilities include managing client intake, onboarding, and service coordination; collaborating with IDOA case managers, CCUs, and MCO care coordinators; ensuring compliance with Illinois Department on Aging (IDOA) Community Care Program (CCP) requirements; communicating with clients, families, and healthcare providers; and building and maintaining referral relationships with community partners.

Qualifications include experience in IDOA CCP, CCU, AAA, Medicaid, or care coordination strongly preferred; background in case management, social work, or home care coordination; RN, LPN, or bachelor's degree in social work, Human Services, Public Health, or related field preferred; knowledge of DON assessments, service authorization, and care planning; experience working with Medicaid, managed care plans such as CountyCare, Meridian, Molina, or Aetna is a plus; and strong organizational, communication, and multitasking skills.
