Job description

Citrus Post Acute is a 24-hour 144-bed skilled nursing facility located in Santa Ana, CA. Our team of dedicated healthcare professionals work to help patients recover from surgery, injury, or from serious illnesses.
Our Rehab team is known to provide the highest quality of care to our residents as our team is full of support and teamwork. We are currently on the search for a Per-Diem PT to join the team in helping us continue to strive for excellence.

About the Position:
The Physical Therapist is responsible for providing evaluations and treatments, developing care plans, and serving as an advocate for the rehab team, patients and their families.
  • Makes evals based on MD's orders/payer authorization and facility/payer guidelines.
  • Develops and executes care plans for residents.
  • Educates staff, patients, and their families.
  • Responsible for documenting daily progress, monthly reports and evals.
  • Screens all individuals who may benefit from therapeutic intervention.
  • Provides PTA's clear directives concerning patients and treatment plans prior to initiation of patient care.
  • Makes appropriate referrals to other members of the rehab team.
Qualifications:
  • PT License or DPT active in CA
  • Minimum of 1 year of Skilled Nursing Facility experience or Long-Term Care preferred
  • Excellent customer service and patient care skills
