Make a Meaningful Impact in Post-Acute Care
Citrus Heights Post Acute is seeking a compassionate and driven Speech-Language Pathologist (SLP) to join our skilled nursing team. In this role, you'll help patients improve communication, swallowing, and cognitive function while working alongside a collaborative interdisciplinary team.
We welcome both experienced SLPs and Clinical Fellows (CFY) —this is an excellent opportunity to grow your career in a supportive, patient-centered environment.
Key Responsibilities
  • Evaluate patients within 24 hours (or 1 business day) of physician referral
  • Develop and implement individualized treatment plans
  • Provide speech therapy services in accordance with physician orders
  • Monitor patient progress and adjust treatment plans as needed
  • Maintain accurate, timely documentation in EMR systems
  • Collaborate with physicians, nursing staff, and rehab team members
  • Participate in care conferences, rehab meetings, and discharge planning
  • Educate patients, families, and caregivers on home programs and ongoing care
  • Provide staff training and in-services as needed
Qualifications
  • Master's or Doctorate degree in Speech-Language Pathology
  • Active California SLP license (required)
  • Strong clinical, communication, and documentation skills
  • Skilled nursing or post-acute experience preferred (not required)
  • Ability to thrive in a fast-paced, team-oriented environment
Certifications & Requirements
  • Current SLP license in good standing
  • Ongoing continuing education per state requirements
  • Experience with or willingness to learn EMR systems
