What You'll Do
As a Clinical Social Worker, you will report to the Manager of Social Services. Provides short-term counseling, crisis intervention and discharge planning services to diverse patient populations. Completes comprehensive psychosocial assessment to determine necessary clinical interventions and assess scope of discharge planning needs. Actively coordinates the planning and implementation of a continuity of care plan. Participates with a multidisciplinary team in the development of individualized treatment plan. Has extensive knowledge of community resources, programs, and agencies.
Clinical Social Worker Job Responsibilities:
  • Conduct comprehensive psychosocial assessments to identify clinical, social, financial, treatment, and discharge planning needs.
  • Provide individual, family, and group therapeutic interventions for patients with complex behavioral health, medical, and psychosocial challenges.
  • Collaborate with interdisciplinary healthcare teams to develop, implement, and evaluate patient-centered treatment and care plans.
  • Coordinate discharge planning, continuity of care, and referrals to community resources, behavioral health services, and financial assistance programs.
  • Advocate for patients and families, facilitating communication, conflict resolution, and informed decision-making throughout the care process.
  • Monitor patient progress, identify barriers to care, and implement solutions to improve outcomes and support successful transitions of care.
  • Maintain accurate, timely, and compliant clinical documentation, assessments, interventions, and case records.
  • Develop and maintain partnerships with community agencies, providers, and support organizations to enhance patient services.
  • Participate in professional development, continuing education, committees, and community outreach activities.
  • Ensure compliance with organizational policies, regulatory requirements, licensure standards, patient safety practices, and mandatory training requirements.

What You Bring
As a Clinical Social Worker, this position requires the skills necessary to provide self-directed social services, including therapeutic individual, family, and group counseling, as well as crisis intervention. The role demands strong clinical assessment and problem-solving abilities, along with a general understanding of illnesses, current treatments, and their psychosocial effects. Candidates must possess thorough knowledge of community agencies, services, entitlement programs, and financial resources available at the federal, state, and local levels to support patients and families in Indiana and other states. Knowledge of state and local adult and child protective legal mandates, family systems, medical insurance, and managed care concepts is also essential. Additionally, the position requires expertise in discharge planning, hospital transfers, home care referrals, and working with patients in acute care settings, including end-of-life care. Successful candidates must be able to maintain professional composure under pressure, work effectively with difficult personalities, and manage crises with sound judgment, initiative, and strong organizational skills. The role also requires the ability to provide age-appropriate care while respecting cultural, ethnic, religious, and economic differences, and to build positive working relationships grounded in empathy and mutual respect. Excellent verbal, written, and listening skills are necessary to counsel patients and families with psychological or psychiatric disorders and to communicate effectively with diverse patient populations, healthcare professionals, hospital staff, and external agencies. Proficiency in basic computer applications and internet research, as well as flexibility, a positive attitude, ethical conduct, discretion, and a strong understanding of patient confidentiality requirements, are also expected. Finally, candidates must demonstrate a commitment to Beacon Health System's mission, values, and performance standards, with a focus on delivering exceptional patient experiences
Required Qualifications
  • The knowledge, skills and abilities as indicated below are normally acquired through the successful completion of a master's degree in social work (MSW), including knowledge of current trends, developments, treatment modalities, etc. for patients with psychiatric and/or psychological disorders.
  • Additional discharge planning or case management experience is desired.
  • A minimum of two years of professional experience performing essential hospital or healthcare social services functions is preferred.
  • Licensure or eligibility for licensure in the State of Indiana as a licensed clinical social worker is required.
  • Must obtain license within 3 years of accrual of required clinical hours necessary to sit for LC SW exam (assuming full time work schedule).
