Position Summary:

We are currently seeking a dedicated and compassionate Registered Nurse Case Manager and a Licensed Practical Nurse to join our Home Health team. The ideal candidate will embrace our mission and provide skilled nursing care while coordinating the overall care plan for hospice patients and their families.

Qualifications:

  • Education: Graduate of an accredited nursing program with a current RN and LPN license.
  • Experience: Minimum of two years of LPN and RN experience required. Previous hospice or home health experience is strongly preferred.
