Position Overview

Therapy Link Solutions is seeking a dedicated and skilled Occupational Therapist (OT) to join our team at the Early Interventional Center located in Fontana, California. Work Environment

The Early Interventional Center provides a supportive and dynamic work environment that prioritizes patient-centered care. You will be joining a multidisciplinary team that includes physical therapists, speech therapists, nursing staff, and social workers, all dedicated to ensuring the best outcomes for our residents. The facility is equipped with modern therapy equipment and resources that facilitate effective treatment and functional rehabilitation.

Key Responsibilities

• Conduct detailed assessments of residents' physical, cognitive, and emotional needs to formulate individualized treatment plans.
• Create and execute engaging therapy sessions that utilize evidence-based practices and therapeutic techniques, focusing on improving daily living skills and functional independence.
• Document patient progress meticulously, adjusting treatment plans as necessary to reflect changing needs and goals.
• Coordinate with interdisciplinary team members, sharing insights and collaborating on a holistic approach to resident care.
• Provide education and support to residents, families, and caregivers on therapy strategies to ensure continuity of care outside of therapy sessions.
• Maintain compliance with all state and federal regulations, ensuring the highest ethical and professional standards in therapy practices.

Patient Population / Client Focus

In this role, you will primarily work with elderly residents, many of whom may be managing chronic conditions, recovery from surgery, or cognitive impairments. Your compassionate approach will not only support their rehabilitation goals but will also help them foster better relationships with caregivers and family members, essential for their ongoing support and recovery strategy.

Qualifications

• A valid Occupational Therapist license in the state of California.
• A Bachelor's degree or Doctoral degree in Occupational Therapy from a regionally accredited program.
• Strong interpersonal and communication skills for effective collaboration with team members and families.
• A commitment to ongoing professional development and education in the field of occupational therapy.

Preferred Experience

• Prior experience in a skilled nursing facility setting is preferred but not mandatory.
• Familiarity with assessment tools and rehabilitation methodologies specific to geriatric care.
• Experience in developing and leading therapeutic group sessions.

Compensation and Benefits

This position offers a competitive salary ranging from USD 120,000 to 130,000 annually, complemented by a robust benefits package to support the well-being of our team.
