Description

Job Summary:

This position is responsible for carrying out the job duties of Children's Mental Health Case Management. The case manager is responsible for assessing client needs, planning for service implementation, coordinating services on behalf of clients, and monitoring services for consistency and efficiency. Case managers are expected to interact professionally with internal and external community providers, perform related work tasks as required or assigned and comply with Minnesota Rule 79 in delivery of Children's Mental Health Case Management services. 

Case managers may be assigned to perform job responsibilities in one or more than one of the counties under the jurisdiction of Minnesota Prairie County Alliance. Under the direct supervision of Director of Children's Services - SPERO and assigned Minnesota Prairie County Alliance Child and Family Supervisor(s).


Supervisory Responsibilities:

  • None 


Duties/Responsibilities:

  • Deliver services according to SPERO and State of Minnesota guidelines for Children's Mental Health Case Management.
  • Maintain a variety of records, including individual client records, time records and travel records in a timely manner. Required to submit these records to other departments as requested.
  • Assure that all client and collateral contacts are documented according to SPERO and State of Minnesota guidelines and that an electronic copy is maintained in the client chart. 
  • Assure that client insurance coverage is requested at intake and updated on an ongoing basis. 
  • Interview client and family, review records and confer with collateral sources including other professionals to identify client needs.
  • Develop client Functional Assessments based on the Diagnostic Assessment and information provided by the family and other collateral sources and review/update every 6 months. 
  • Ensure the Clinical Supervisor receives a copy of all clients' Comprehensive Evaluations, both initial and others obtained while case is open. Ensuring eligibility is determined at least once every 3 years. 
  • Assure that required Clinical Supervisor signatures are obtained on the ITP, FA, and Out of Home Placement Plan (as needed). 
  • Develop an Individual Treatment Plan (ITP) within 60 days of opening the CTCM case including measurable goals and objectives based on the Functional Assessment and recommendations from the Diagnostic Assessment. 
  • Notify and consult client's assigned mental health professional, Minnesota Prairie County Alliance supervisor, and SPERO Clinical Supervisor when client experiences a significant change in status. 
  • Review and update the Individual Treatment Plan (ITP) according to changes in client status, or at least every 6 months, documenting clients' progress during the previous 6 months. 
  • Coordinate referrals of client and family to community resources needed to meet client needs and the ITP goals and objectives.
  • Develop and/or review Crisis Response Plans /Safety Plans with clients and families semi-annually or more frequently as needed.
  • Assist other Case Managers with urgent tasks as needed. 
  • Communicate promptly with office co-workers and supervisors regarding schedule and schedule changes.
  • Comply with Clinical Supervision and Continuing Education requirements as outlined by the Minnesota Department of Human Services for Children's mental health case management.

Requirements

Required Knowledge, Skills, and Abilities: 

  • Staff must successfully pass all background and credentialling requirements for facility and position. 
  • Staff must have a desire to work with children and families in their home, in the community or at the office.
  • Staff must be able to demonstrate independence and initiative in completing job responsibilities as well as effective verbal and written communication skills.
  • Staff will have knowledge of child and adolescent developmental stages and child and adolescent mental health diagnosis. 
  • Staff must have knowledge of evidence-based practices when working with children/adolescents and family systems, de-escalation strategies and techniques.
  • Staff must have a valid driver's license/ auto insurance/ reliable vehicle / be willing to provide transportation for clients.


Education and Experience:

  • MINIMUM QUALIFICATIONS: Bachelor's degree in one of the behavioral sciences or related fields, including but not limited to social work, psychology or nursing from an accredited college or university.
  • PREFERRED QUALIFICATIONS: Bachelor's degree as above, four (4) years' experience in human services field, two (2) of those years as a children's mental health case manager.


Physical Requirements: 

  • Must be able to transport clients in the community or to appointments as required.
  • Complete all paperwork, reports, and charting in a professional and timely manner.
  • Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in scheduled supervision.
  • Complete training as assigned.
  • Sit at a desk for extended periods of time and perform long hours of work sitting at a computer.
  • Maintain regular work hours as determined and assigned. 


Work Environment:

  • Understands the confidential nature of organizational information and maintains confidence.
  • Experience in the use of some office equipment such as copy machine, fax machine, postage machine, programmable telephone system, EMR system, etc.
  • Familiarity with standard computer functions, including keyboarding, and applications including e-mail.
  • Work responsibilities may be completed in the client's home, various community locations or the agency office.
