About the Health Care Home Program
The Health Care Home program integrates care for chronic health conditions within a Community Mental Health Center setting. The program supports individuals in accessing needed health services, managing chronic conditions, and improving overall health through ongoing monitoring, early intervention, and care coordination. Health Care Home promotes wellness, healthy lifestyles, and preventive care while using a population health approach to improve outcomes for the individuals served.
What You'll Do
  • Champion the integration of physical and behavioral health services, prevention, and wellness through ongoing quality assurance and continuous improvement initiatives.
  • Direct member enrollment, discharge, and transfer processes to ensure timely, accurate, and compliant transitions of care.
  • Oversee the day-to-day operations of the Health Care Home (HCH), including the preparation and submission of all required monthly reports to the state.
  • Participate in health education and outreach activities-such as smoking cessation, nutrition, disease prevention, and medication adherence-when appropriately credentialed.
  • Oversee population health monitoring efforts to identify gaps in care and implement strategies to improve health outcomes.
  • Analyze state population health reports using critical thinking to identify opportunities, barriers, and gaps in care, and develops actionable solutions and strategies aligned with the Health Care Home scope and agency mission.
  • Prepare and provide oversight of the annual Health Care Home program budget.
  • Develop, implement, and maintain departmental policies to ensure Health Care Home and Wellness programs remain compliant with requirements set forth by the Missouri Department of Mental Health Coalition, ReDiscover, and CARF.
  • Provide leadership oversight of Health Care Home and Wellness Center staff, including performance management, professional development, and disciplinary action as needed.
  • Ensure timely and accurate completion of all monthly reports required by the Department of Mental Health Coalition.
  • Ensure agency compliance with Health Care Home program guidelines established by the Department of Mental Health Coalition, including staffing requirements and enrollment limits.
  • Attend and actively participate in State Health Care Home meetings as outlined by the Department of Mental Health Coalition.

Requirements of the Position
  • Graduation from an accredited nursing program with a Registered Nurse (RN) license or Master's Degree (MSN) required
  • Active, unrestricted Missouri RN license required.
  • Ability to assess an individual's overall health using knowledge and skills grounded in psychology or related mental health field is required.
  • A minimum of five (5) years' clinical experience in direct service provision to adults and children in a health setting is required.
  • A minimum of two (2) years' clinical experience in a mental health setting is required.
  • A minimum of three (3) years' supervisory and management experience is required.
  • Valid driver's license, proof of insurance, and a reliable vehicle.
  • Strong communication and customer service skills
  • Comfort with electronic health records and intermediate computer proficiency (Microsoft Office products and use of email).
