Job description

We are looking for a motivated and driven Children's Mental Health Practitioner to make an impact on the lives of children's that have severe emotional disturbances. You will work with clients and their families to help coordinate and obtain services that are needed to help improve their mental health and overall wellbeing.ย Services are child centered, family focused, and community based. Primary services provided by the mental health practitioner are case management and skills which include assessment, skills training, service plan development, referral, linkage, monitoring, coordination of care.

Responsibilities

  • Assess client and family needs by collecting, evaluating, and documenting information about the clients situation, personal and family needs, and personal and family strengths.
  • Develop service plans in cooperation with the family,ย other providers, while developing goals and expectations.
  • Coordinate and make referrals for needed services in order that each client may function to the best of her/his ability.ย 
  • Provide face to face skills training services to clients.
  • Maintain knowledge of city and/or county mental health, medical, financial, housing and other human and family services agencies.
  • Complete and maintain data privacy, intake and other required documentation.

Requirements and Qualifications

  • Bachelorโ€™s degree in social work, psychology or a related human services field.
  • 1 year of experience providing services to individuals with a mental illness or substance use disorder required.
  • Must qualify as a Mental Health Practitioner in the State of MN
  • Experience working with severe emotional disturbancesย inย children is preferred.
  • Valid driver's license and proof of auto insurance.

Pay: $25.11 - $30.13 per hour. Compensation is based on years of experience.
