RN - Hospice Liaison (Hospital-Based) | Travel/Contract
Position Overview

We are hiring an experienced Hospice RN Liaison to support hospital-based hospice referrals, assessments, and admissions. This role works closely with hospital care teams, physicians, and families to determine the appropriate level of hospice care and facilitate smooth transitions.


Key Highlights
  • Setting: Hospital-based (liaison + admissions)

  • Schedule: Monday-Friday (No weekends, No call, No holidays)

  • Traveler Friendly: First-time travelers accepted

  • Patient Load: ~2-3 patients/day

  • Coverage: Assigned to 1-2 hospitals per day


Facilities Covered (Aurora Network)

May include:

  • Aurora Sinai Medical Center

  • Aurora St. Luke's Medical Center

  • Aurora West Allis Medical Center

  • Aurora Medical Center Grafton

  • Aurora Medical Center Hartford

  • Aurora St. Luke's South Shore

Role Responsibilities
  • Monitor referral queues from hospitals and SNFs

  • Perform bedside hospice assessments

  • Recommend appropriate level of hospice care:

    • Routine hospice

    • General inpatient (GIP)

    • Respite care

  • Admit patients to hospice (including in hospital setting)

  • Educate patients/families on:

    • Hospice philosophy

    • Benefits and expectations

  • Collaborate with:

    • Physicians

    • Case managers

    • Hospital care teams


Patient Population
  • Adult patients with:

    • Heart failure

    • Cancer

    • End-stage disease processes


Requirements (MUST HAVE)
  • Experience:

    • Minimum 2 years hospice experience

    • Hospice liaison experience preferred (not mandatory)

  • Certifications & Credentials:

    • BLS

    • Valid Driver's License

    • Auto Insurance & Personal Vehicle required

  • Technical Skills:

    • Epic experience REQUIRED

  • Strong end-of-life symptom management (pain, SOB)

  • Expertise in hospice medications & education

  • Confident physician communication & collaboration

  • Deep understanding of:

    • Hospice levels of care

  • Shift: Monday-Friday, 8:00 AM - 6:00 PM

  • Rotating day off each week

  • Occasional block scheduling possible (not guaranteed)

  • No weekends / No call / No holidays
