Healogics is the largest provider of advanced wound care services in the United States, treating more than 300,000 chronic wound patients annually across over 600 sites. With an aging society, obesity and diabetes on the rise, and an uptick in surgical procedures, the number of patients with non-healing wounds that would benefit from expert care is dramatically increasing. As a result, the company is working to provide our differentiated, quality outcomes to as many patients that would benefit through our out-patient clinic partnerships.
The Chart Management Coordinator II (CMC) performs a variety of Health Information Management (HIM) administrative duties to include medical record retrieval, storage, and maintenance. Additionally, the individual in this role assists with claim scrubs, rejections, and related billing operations to ensure correct claims submission.
All Healogics employees must perform their job responsibilities according to all Healogics policies, Hospital policies, as well as to accrediting organizations, federal and state regulation, and to the Centers for Medicare and Medicaid Services (CMS) guidelines, as applicable.
Essential Functions/Responsibilities:
  • Pulls and retrieves medical records and associated encounter documents from disparate Hospital Electronic Health Record (EHR) systems for coding and billing processing
  • Manages and maintains Hospital EHR accesses and assures HIPAA and information security compliance to each hospital's business agreement and access privileges
  • Works with our Internal Practice Management Software for Medical Record retrieval
  • Coordinates and completes requests for the release of medical records to both internal and external customers, patients, and agencies
  • Compiles medical record requests for insurance carrier audits and appeals
  • Coordinates attorney and subpoena requests with Healogics Office of the General Counsel
  • Troubleshoots missing and/or incomplete medical documentation
  • Identifies corrective actions and coordinates directly with providers, Program Directors, Revenue Cycle Managers, and other Operations resources
  • Reviews E-Print Chart Management Report daily for scanning and uploading of medical documentation (EHR) to our Practice Management Software systems for billing
  • Assists and research coder identified "Deficiencies" within the LightSpeed Vericode platform; facilitates corrective actions to complete the coding of encounters
  • Responds to inquiries from Program Directors and other Operations personnel with regard to chart management, EHR systems, etc.
  • Maintains a thorough knowledge of Chart Management processes and Practice Management
  • Performs billing functions to review and take necessary actions to resolve billing errors ensuring clean claim submission
  • Works directly with payers, internal and external customers, toward effective and efficient claims resolution
  • Communicates with providers/wound centers regarding incomplete documentation
  • Responds to Customer Service inquires
  • Meets departmental productivity and quality assurance standards in accordance with departmental policies
  • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards
  • Actively participates in creating and implementing improvements
  • Performs charge corrections per departmental requests
  • Maintains a thorough knowledge of billing and collection requirements for payers assigned
  • Performs other duties as required

Required Education, Experience and Credentials:
  • High School Diploma or General Education Degree (GED)
  • Minimum of three (3) years' medical office experience in a Physician's office preferred
  • Combination of experience in other settings and advanced education will be considered

Required Knowledge, Skills and Abilities:
  • Excellent attention to detail
  • Ability to work independently as well as in a team setting
  • Ability to multi-task and follow-up on tasks
  • Must be able to demonstrate teamwork and display excellent communication skills
  • Microsoft Excel and Word experience

The hourly rate for this role is 20.00 - 22.50 USD Hourly
