As a licensed Physical Therapist in our home care setting, you will work closely with a diverse patient population, ranging from those recovering from surgeries and injuries to individuals with chronic conditions and disabilities. Our collaborative work environment allows you to engage with an interdisciplinary team of healthcare professionals, including physicians, occupational therapists, and social workers, ensuring comprehensive care tailored to each patient's unique needs.

• Conduct thorough evaluations and assessments of clients to formulate effective and individualized treatment plans based on established guidelines and best practices.
• Implement specific therapeutic interventions to address mobility issues, pain management, and functional limitations, with the goal of restoring independence.
• Document evaluations and treatment sessions accurately within our electronic medical records system, ensuring compliance with regulatory standards and appropriate billing practices.
• Perform screenings for various conditions, including lymphedema and fall risks, to determine the necessity of rehabilitation services for patients.
• Collaborate with interdisciplinary teams to discuss patient outcomes and adjust treatment plans as necessary to optimize recovery and care.
• Participate in departmental education initiatives, including supervision of interns and contributions to presentations and publications, fostering a culture of learning and growth.

Your role will focus on patients dealing with a variety of challenges including physical injuries, cognitive impairments, psychosocial dysfunctions, developmental disabilities, and chronic illness. By implementing customized treatment approaches, you will have the opportunity to significantly improve their quality of life, enhance functionality, and prevent long-term disabilities.

• Active Florida Physical Therapy License.
• A minimum of one year of clinical experience, preferably in an acute care hospital or similar healthcare settings.
• Proficiency in trigger point therapy techniques and a solid understanding of human anatomy and physiology.
• Experience working with diverse patient populations, including children and adults, ensuring that treatment is appropriately tailored.
• Comfortable navigating both clinic and home environments to provide necessary care.

• A Master's or Doctorate degree in Physical Therapy.
• Familiarity with hospital medicine practices and competencies in providing inpatient care.
• Strong interpersonal skills and the ability to collaborate effectively in a team environment.
