Role Description

Jaan Health is seeking dynamic and motivated Certified Medical Assistants (CMA) Clinical Care Coordinators to join our care team and will report to the Clinical Operations Manager. They will support the development of patient-centered, team-based care, as well as support primary care physicians (PCPs) and practices in managing their panel of patients using a population management informatics tool. This role focuses on high-volume patient outreach through calls, texts, and care platform messaging, while ensuring accurate documentation and seamless workflow with the care team. In addition, this role will support key operational functions such as member enrollment, staff training, and client operations, bridging the gap between clinical expertise and operational execution.

Key Responsibilities:

  • Conduct high-volume patient outreach via phone, text, and digital platforms.
  • Document patient interactions and care management activities in EHRs (EPIC, Cerner, Allscripts, eCW, etc).
  • Support LPNs, RNs and providers with care management activities such as: intake coordination, appointment scheduling,medication refills, follow-ups, and escalate clinical issues to LPNs/ RNs as appropriate.
  • Assist with patient onboarding and enrollment processes, partner with client operations teams to ensure seamless delivery of services.
  • Deliver training sessions on care protocols, systems, and operations to care management teams.
  • Adapt to evolving workflows, technology, and program demands
  • Contributes to a positive experience for patients and families through courteous telephone and digital interactions, accurate and expeditious routing, as well as referral to appropriate clinical staff when necessary.
  • Perform all job functions in compliance with applicable federal, state, local and company policies and procedures.

Required Qualifications:

  • Certified Medical Assistant required.
  • Working PST Time Zone
  • Minimum of 3 years-experience in care management at a primary care setting or similar specialty. Experience in population health preferred.
  • Excellent communication and technology skills.
  • Prior use of electronic health records and other health care information systems required.
  • Significant experience within a primary care setting (chronic care preferred, hospital, medical groups).
  • Proven problem-solver, comfortable with high-volume outreach, and ability to multi-task.
  • Must have access to reliable internet and have a secure/private environment to work
