Referral Development | East Central Ohio

About CCH Healthcare

CCH Healthcare is a growing provider of skilled nursing and rehabilitation services. We are seeking a resultsdriven SNF Marketer to support two affiliated skilled nursing facilities in East Central Ohio. This role focuses on census growth, referral development, and admissions conversion through strong relationship building.


Position Overview

The Skilled Nursing Facility Marketer serves as the primary referral and marketing partner for two SNFs. This role is responsible for building census, managing referrals, and strengthening hospital and community partnerships. Ideal candidates have postacute or SNF marketing experience and a proven ability to convert referrals into admissions.


Key Responsibilities
  • Serve as dedicated marketer for two SNFs in East Central Ohio
  • Build and maintain referral relationships with:
    • Hospital case managers and discharge planners
    • Social workers and care coordinators
    • Physicians and physician groups
    • Home health, hospice, and community partners
  • Generate, track, and follow up on referrals to drive consistent census growth
  • Educate referral sources on services, payer acceptance, and admissions criteria
  • Conduct regular inperson sales and market visits
  • Partner closely with Admissions, Administrators, DONs, and Corporate Leadership
  • Monitor census, payer mix, and referral trends
  • Maintain accurate referral and census reporting

Qualifications
  • SNF, postacute, or healthcare marketing experience preferred
  • Proven success in SNF census development
  • Strong knowledge of Medicare, Medicaid, Managed Care, and commercial payers
  • Excellent communication and relationshipbuilding skills
  • Organized, selfmotivated, and resultsdriven
  • Ability to manage responsibilities across multiple facilities
  • Valid driver’s license with local travel in East Central Ohio
