RN Hospice Clinical Liaison

This is a strong opportunity for a clinically grounded RN who enjoys relationship-building, patient and family education, and being out in the community as a trusted hospice resource. The right person will bring both clinical credibility and a warm, professional approach to helping referral partners, patients, and families better understand hospice care.

This role works closely with referral sources, healthcare partners, patients, and families to provide education, answer clinical questions, and support a smooth transition into hospice care when appropriate.

This is a good fit for an RN who is comfortable in the field, enjoys developing professional relationships, and can represent hospice services with compassion, confidence, and clinical knowledge.

Position Responsibilities:

  • Build and maintain relationships with hospitals, physicians, discharge planners, case managers, skilled nursing facilities, social workers, and other healthcare referral sources.
  • Provide hospice education to referral partners, patients, and families in a clear, compassionate, and clinically appropriate manner.
  • Serve as a clinical resource for hospice eligibility, services, care philosophy, and transition planning.
  • Develop new referral relationships while continuing to support and strengthen existing accounts.
  • Participate in regular outreach throughout the East Mesa market to support census growth and community awareness.
  • Help patients and families understand hospice options during sensitive and important decision-making moments.
  • Communicate effectively with internal clinical and operations teams to support timely, appropriate admissions.
  • Represent the organization with professionalism, empathy, and a patient-centered approach.

Requirements

  • Current, unencumbered Registered Nurse license required.
  • Hospice, palliative care, home health, hospital, or post-acute experience strongly preferred.
  • Prior healthcare sales, marketing, liaison, intake, admissions, or referral development experience preferred.
  • Hospice or palliative care marketing/liaison experience is a strong plus.
  • Strong communication, relationship-building, and follow-through skills.
  • Ability to work independently in the field while staying connected to the broader team.
  • Comfortable speaking with physicians, hospital staff, facility leadership, case managers, discharge planners, patients, and families.
  • Reliable transportation, valid driver’s license, and current auto insurance required
