Job description

Medical Respite (Part Time; Weekends; 24 hours)

Shift: 3rd Shift; Weekends

Medical Respite care is acute and post-acute care for persons experiencing homelessness who are too ill or frail to recover from a physical illness or injury on the streets but are not ill enough to be in a hospital.

This is a short-term residential program that allows individuals experiencing homelessness the opportunity to rest in a safe environment while accessing medical care and other supportive services.

POSITION SUMMARY :

  • Ensure general safety and structure of the program by monitoring resident activities as they pertain to shelter rules and policies.
  • Additionally, ensure general safety of the residents by identifying any issues in the facility that are in need of repair and alerting your Supervisor as soon as possible.
  • Establish rapport with residents and establish a sense of trust.
  • Maintain general operation of the program by being available to residents as needed/requested.
  • Complete routine circuits of the program space for each shift and record in the log, with any significant findings noted.
  • Record residents coming in after curfew.
  • Provide crisis intervention with the assistance of CCA Emergency Services.
  • Respond to resident concerns (provide concern form if needed).
  • Record all incidents on a typed incident form as required.
  • Attend required trainings.
  • Handle all resident information confidentially and with respect.
  • Perform all shift cleaning and maintenance responsibilities.
  • Any duties as assigned by supervisory staff.
  • Coordinate with the Thundermist Care Team to ensure that we are mutually up-to-date on the strengths-needs-abilities-preferences of each resident.
  • Coordinate with the CCA programs based upon resident need, facilitating appropriate referrals for behavioral health treatment, peer recovery supports, housing stabilization services and/or basic needs.
  • Assist with appropriate referrals at the time of discharge.

Education Requirements: High School Diploma / GED or 1 year relevant work experience.
