Job Summary
The Division of Infectious Diseases is searching for a Navigator to join the Bluegrass Care Clinic and Ryan White Programs. The Navigator position will provide support to patients and their family members to navigate appointments, services, resources, and schedules related to the care provided within the division of Infectious Diseases. This position will work with patients and caregivers to facilitate communication between our physicians, clinic staff, and care providers in the continuum of care to identify strategies and resolve barriers to healthcare. The Navigator will address and resolve concerns and issues so that treatment plans are most effective. Ultimately, the Navigator works to promote a comprehensive positive healthcare experience to patients, their families, referring providers, and UKHC Healthcare staff.
The Navigator will also serve as support staff to the Ryan White medical case management team, primarily responsible for triaging walk-in clients in a timely manner and coordinating case management services for such clients. This includes monitoring the walk-in phone line and EPIC pool messages, meeting with walk-in clients, and providing transportation assistance (Uber, Yellow Ride, gas cards, bus passes, and Wheels vouchers) and basic needs support (food and hygiene supplies) according to established protocol. The Navigator will assist with enrollment and KADAP recertification processes, including preparing recertification packets and will refer clients to other support teams when appropriate. This role allows the medical case management team to focus on intensive case management, follow up, and scheduled appointments.
Skills / Knowledge / Abilities
Use of electronic medical record, good problem solving skills. Excellent communication and customer service skills. Knowledge of and experience working with community based resources providing services to the identified patient population.

Bachelor's degree in Social Work, Social Services, Health Education or Human Development preferred.
