Case Manager Opportunity!
This full-time opportunity offers 40 hours per week, full benefits, and a day shift schedule providing care.
The Case Manager is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care.
Granite Hills Hospital is a newer resource for Behavioral Health in Wisconsin that offers high-quality, patient centered care for the greater Milwaukee community. With a wide range of treatment for children, adolescents, adults and older adults, our mission is to provide much needed outcome-focused care for those who need it most. Our recently opened 120-bed facility features evidence-based treatment modalities and a multi-disciplinary team led by expert medical professionals.

Qualifications
Education: Master's Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.
Experience : Minimum of three (3) years' experience in a psychiatric health care facility with direct experience in Case Managment and discharge planning.
Additional Requirements : Crisis Prevention Certification Trainin gs (Handle with Care ) within 30 days of employment and prior to assisting with a restraining procedure . Certification training is offered during new hire orientation.
