Job description

SUMMARY: Under the direction of the Supervisor Transition of Care, provides clinically based case management to support delivery of effective and efficient patient care. This position partners with the Case Management and multidisciplinary teams to identify appropriate utilization of resources and to ensure reimbursement.
EXPERIENCE: Minimum 2-3 years' experience in Case Management or related healthcare field.
SPECIAL SKILLS: Excellent communication, interpersonal, organizational and presentation skills.
OTHER: Computer skills.
LICENSE/CERT: Required: RN (Registered Nurse) State of Missouri Preferred: ACM (American Case Management) within 1 year of hire
EDUCATION: Required: Bachelors - Nursing
