Job description

Per Diem -- Days / Evenings, Case Manager, Intensive Crisis Residence
St. Vincent's Hospital Westchester in Harrison, New York is seeking a dynamic, experienced Case Manager for a full time position in our Intensive Crisis Residence. The ICR will be a licensed, voluntary, short--term residential treatment program for adults experiencing a psychiatric crisis, providing treatment and supports to help residents return to their previous level of functioning in the community.
Reporting to the Program Director and working in collaboration with a multidiscipline team, the Case Manager provides crisis stabilization and community linkage services in a fast-paced environment. Ideal candidates will have excellent engagement and assessment skills, and the ability to provide direct case management services that support crisis stabilization and engagement in ICR services. In collaboration with the resident, the Case Manager facilitates aftercare planning, linkages to community services and post-discharge follow up to support a successful transition back to the community.
Requirements: A bachelor's in social work, psychology or a related field and at least one year working with adults with serious mental illness.
Pay: 1199 collective bargaining
