Description
Summary: This unique position's primary function is to fulfill Catholic Charities commitment to serve individuals and families in an inclusive and holistic manner. An integral part of the Case Management team , the Case Manager will be based in the Yonkers and travel within Westchester County as needed . The position provides direct assistance for those in need through case assistance and case management services . This position will have a particular focus on New Yorkers who arrived in the United States on or after January 1, 2022, as part of the Assistance to Migrant Program.
Salary: $30.21 per hour, this is a non-exempt hourly position based on a 35 hours work .
Location: 204 Hawthorne Avenue, NY, 10705.
Essential Duties and Responsibilities include the following :
  • Provides Case Management to a broad population of all individuals and families for a variety of needs.
    • Provides effective engagement, assessment, service planning, and follow-up using a strength 's based and client- centered approach.
    • Provides crisis interven tion in emergencies around shelter, food, clothing, and other basic needs through the provision of financial assista nce, goods, advocacy, benefits access, and access to relevant resources.
    • Conducts thorough assessments beyond the presenting need and develops a service plan that addr esses immediate need and plans toward self-sufficiency.
    • Develops a comprehensive understanding of the complex social service system and helps clients navigate these systems accordingly .
    • Conducts outreach in the community.
    • Assists in applying for public and private benefits.
  • Provides Resource and benefits access through direct assistance and/or referral.
    • Develops knowledge about and maintains collaborative relationships with community agencies, other Catholic Charities Departments, Divisions, Affiliates and Archdiocesan entities.
    • Develops a capacity to provide or ensure access to a wide range of social service benefits and helps develop and maintain a network of high-quality resources.
  • Fulfills operational and other duties.
    • Enters a ll information into the ECM database , meets program deliverables in a timely and efficient manner, and understands the significance of documentation to funders and other stakeholders.
    • Completes all requests for financial assistance and gathers relevant supportive documentation in a timely, efficient, and thorough manner.
    • Actively participates in weekly supervision.
    • Attends and participates in monthly Department meetings, community meetings, trainings, and other agency-related activities.
    • Assists at other sites as needed.
    • Other duties as assigned.

Position Type and Expected Hours of Work:
This is a f ull-time, non exempt position. Days and hours of work are generally 9:00 AM to 5 PM Monday - Friday . Additional hours may be required to meet program deadlines, or client needs. There may be periods when work hours vary.
Hourly range: $30.21
Working conditions and physical demands required :

  • Travel to various pantry sites, outreach, and staff meetings . The ability to travel using reliable transportation, travers ing streets, ascend ing /descend ing stairs, and exposed to outside weather conditions.
  • Traverse neighborhoods that may require a high level of alertness and awareness of surroundings.
  • Work with clients who require consistent and comprehensive support , some under potentially stressful conditions.
  • Remain in a stationary position at a workstation an d use a computer approximately 6 0% of the time.

Qualifications
Education and/or experience required:
  • Bachelor's degree required or at least four years of experience in the social services field .
  • Bilingual English/ Spanish required.

Skills, Licenses, and/or competencies required:
  • Excellent interpersonal skills ; t he ability to work well independently and as part of a team.
  • Knowledge of social service delivery systems and local resources .
  • Excellent organizational, time-management and communication skills .
  • WIFI access and an ability to successfully navigate remote technology.
