Qualifications
Required:
  • Bachelor’s Degree in a human services field
  • Valid driver’s license and reliable transportation
  • Ability to pass NCIC/CIB and APS/CPS background checks
  • Three professional references
  • Strong written and verbal communication skills
  • Proficiency with Outlook, Word, Excel, and PDF platforms
  • Professionalism and strong customer service skills
  • Ability to work independently with minimal supervision
  • Ability to work evenings and weekends as needed
  • Experience with virtual platforms
  • Strong time management skills
Preferred:
  • 2–4 years of case management and/or behavioral health experience
  • 1–2 years of experience leading teams
  • Experience providing direct services to children and families
Key Responsibilities
  • Provide wraparound services to a caseload of up to 15 families
  • Coordinate supports, resources, and services using a strengths-based approach
  • Facilitate team meetings with youth, families, and multidisciplinary partners
  • Collaborate with MCO care managers and community resources
  • Conduct and administer the Child and Adolescent Needs and Strengths (CANS) assessment
  • Monitor service implementation and ensure families receive identified supports
  • Lead planning from engagement through transition and discharge
  • Conduct home visits according to each family’s individualized plan
  • Identify service gaps and collaborate with community providers to address needs
  • Maintain accurate, timely, and compliant documentation
  • Participate in scheduled supervision and coaching
  • Maintain client records in alignment with agency standards and regulatory guidelines
Physical Requirements
  • Ability to walk up and down stairs

  • Ability to kneel, bend, squat, and stand
