Job description

Community Care is hiring a Case Manager. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. Community Care's mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule and a $8,000 SIGN-ON BONUS. Position located in Dane Co.
Case Manager Job Responsibilities:
  • Manages an assigned caseload of program members in collaboration with an RN Case Manager
  • Completes comprehensive member assessments, member centered care plans, & functional screens
  • Provides ongoing care coordination and quality assurance monitoring of services
  • Maintains required documentation in adherence to established standards.

Case Manager Job Requirements:

Education: Bachelor's Degree in a Human Services or closely related area required. A minimum of three (3) years' experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.

Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.

Certification: Eligibility for long-term care functional screener certification is required

Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
Case Manager Job Schedule:
  • Monday to Friday, 8:00 AM-4:30 PM
