Registered Nurse (RN) – Home Care Case Manager (Part-Time)

CareSphere Homecare is excited to offer a rare opportunity for an RN to work part time.The role is Office based out of our Philadelphia Location with local travel, typically 3-4 home visits per day for brief assessments.

Schedule:

10hrs a week to build up to 20 FLEXIBLE

Day Shift— No holidays, and No on-call.

About the Role

  • This is a Non-Skilled Case Management position—no clinical care required.
  • Schedule and perform Q60 day HHA Supervisory/Recertification visits
  • Schedule and perform Start of Care assessment visits
  • Ongoing Case Management for client and HHA oversight
  • Evaluate the client status and the skills of staff members
  • Complete timely and accurate clinical assessments within 3 days of the visit
  • Complete timely and accurate 485/ Plan of Care for patients within 3 days of the visit
  • Provides patient, family, or caregiver education

The role is Office based out of our Pittsburgh Location with local travel, typically 3-4 home visits per day for brief assessments.

  • Active RN License in Pennsylvania
  • Valid Driver’s License and reliable vehicle
