RN Case Manager | Home Visits, Grand Rapids Area

Join Our Team as an RN Case Manager for Home Visits in the Grand Rapids Area. Atrio Home Care, the partnership of Holland Home, Brio and Resthaven, is seeking a compassionate RN Case Manager to join our Grand Rapids Team. This position will involve providing direct care to clients within a 35-mile radius of Grand Rapids.

Responsibilities

  • Perform initial and comprehensive assessments and deliver compassionate care to clients
  • Collaborate with physicians and other care team members to develop a Plan of Care
  • Promotes client and family/caregiver independence through counseling and education
  • Coordinate client discharge planning
  • Identify barriers to desired outcomes and evaluates outcomes of care
  • Prepare clinical documentation, progress notes in a timely manner using iPads for theEMR system
  • Participates in on-call rotation

Location: Grand Rapids, covering a 35-mile radius around GR

Schedule/Shift

  • Monday - Friday
  • Day shift 8-5 pm, with flexibility in scheduling to help you maintain a healthy work/life balance
  • Weekend rotation; every 6/8 weekends

Compensation

Qualifications

  • Licensed RN in Michigan; BSN preferred
  • Valid Michigan driver's license, proof of auto insurance, and reliable transportation for home visits

  • Passion for delivering high quality care to older adults
