POSITION SUMMARY

– RN Case Manager

POSITION DUTIES

– Coordinates patient care by assessing needs, developing and implementing individualized care and discharge plans, collaborating with interdisciplinary teams, and ensuring appropriate resource utilization to support optimal recovery and transition to the next level of care.

MINIMUM REQUIRED QUALIFICATIONS
  • MI RN license
  • BLS
  • 1+ year case manager experience in an acute hospital setting
PREFERRED QUALIFICATIONS

– Acute rehab experience

LENGTH OF ASSIGNMENT

– 13 weeks

SHIFT / HOURS PER WEEK

– M-F 8:00am-4:30pm
