Hospice Registered Nurse Case Manager (RNCM)

Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Hospice of the West is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday, 8 am - 5 pm

Territory: Scottsdale/Fountain Hills Area

In this rewarding role, you'll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Qualifications
  • Hold current unencumbered license as a Registered Nurse.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
  • Must be computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification if required by state.
  • Current driver's license, vehicle insurance, and access to dependable transportation.

$83,000 - $94,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate's relevant experience, qualifications, and the specific responsibilities of the position.
